 Women's Preventative Health Eligibility is based on financial criteria. Call our Eligibility Staff at (828)695-4445 with the following information:
  • Social Security #
  • Name/Address/Phone
  • Employer/wages (last check stub)
  • Medical expenses (within 6 months)
  • Insurance preiums (within 6 months)
  • Insurance, Medicaid, Medicare into
  • Other income
