by
visiting the Government Center, at 100A
Southwest Boulevard in Newton, to submit
an application and pay the fee or by downloading
the application
(PDF) and mailing the completed
form to the Environmental Health
Division at PO Box 389, Newton, NC 28658,
together with the applicable fee listed
below.
Contact the Environmental Health Specialist
for your area between 8 am and 9 am after
the application has been submitted to
schedule a day for the sample to be taken.

specialist to call).
Bacteriological sample results take
4-5 days, chemical sample results
take 3-4 weeks and all other sample
results take 1-2 months.
If a bacteriological sample is for
a loan closing, allow 3-4 weeks for collection,
transport, and analysis of sample, disinfection
of well if sample is positive, and resample.
Keep a copy of your completed application
until the process is complete.
