CAROLINA ACCESS

To provide a more efficient and effective health care system by linking Medicaid recipients with a primary care provider who delivers and coordinates health care needs, and implements a policy for use of emergency department services.

Mandatory enrollment for individuals receiving full Medicaid benefits (Blue Card).  Optional enrollment for those receiving Medicaid for Pregnant Women (MPW) & those receiving both Medicare and Medicaid benefits.

Referral from ACH validating that Medicaid eligible adult needs extensive or total assistance with eating, tolieting and/or ambulation at least 3 times weekly.

Medicaid program in which a variety of in home and community services provide an alternative to nursing home placement for the elderly or disabled adult. Disabled children who are at risk of institutionalization and are either nursing home or hospital level of care may also receive CAP services.

Individual must be receiving Medicaid, and CAP/DA clients must need the same level of care as someone in a nursing home. Cap/C clients must need either the same level of care as someone in a nursing home or a hospital.

Depends On Funding Source: Block Grant - 60 Years and older who are in need of Level II or Level III personal care. Block Grant - Individuals 18 years of age or older who are unable to perform their own personal care and home management tasks, and who have no responsible person available to assist them.

STATE COUNTY SPECIAL ASSISTANCE IN-HOME PROGRAM FOR ADULTS (SA/IH)

Provides a cash supplement to help low income individuals who are at risk of entering an Adult care Home (ACH), and who would like to remain at home. SA/IH provides an alternative to placement in an ACH for individuals who could live at home safely with additional support services and income. The program can help with living expenses such as food, shelter, clothing and other daily necessities.

Individual must be receiving Categorically Needy Medicaid (at or below the Federal Poverty level for the aged, blind or disabled), be at minimum age 18, have a doctor's verification of need for ACH or group home care, have a desire to remain (or return) to their home*. Must be a resident of a county providing SA/IH, US Citizen or Qualified Alien.

*Current ACH residents may apply for SA/IH and return to their homes if approved.

No Cost to the recipient, but the SA/IH income may affect Food Assistance Allotment.

Information Needed to Apply for Services-Verification of all income sources and countable resources such as savings and checking accounts, CD's, IRA's, stocks, bonds, Life insurance, real and personal property, place application for all potential benefits (SSI, pensions, VA pensions, etc). Provide resource information for 36 month lookback to identify any transfer of assests. If income below SSI limit, must appply (and be awarded) SSI.
