    The Community Services Program is comprised of the Intensive In-Home (IIH) teams and Mental Health Targeted Case Management (TCM) professionals.

    Intensive In-Home Services

    The IIH teams work with children and their families primarily in the home to stabilize the home environment, resolve crises, assist in skills-building of the child/adolescent, increase parenting skills, and coordinate other needed services/resources. Each 3-member team led by a licensed clinician works in partnership with the family to individualize treatment to fit the family’s needs in order to prevent out-of-home placement or assist with transitioning from out-of-home placement. The service is time-limited, more intensive at the beginning of treatment and decreases over time as the child and family’s skills develop. The IIH teams also provide “first responder” crisis response on a 24/7/365 basis to the child/family as needed.

    Targeted Case Management

    The TCM professionals provide case management to arrange, link or integrate multiple services, as well as assessment and reassessment of the youth’s need for services. The TCM professionals keep informed of community services/resources and assist the child in accessing needed services and monitor the provision of services to ensure the child’s needs are being met and treatment goals are addressed. They provide coordination of movement across levels of care and assist with discharge planning.
